CPR: 100–120/min, ≥5 cm, full recoil, minimize pauses; 2‑min cycles.
Shockable (VF/pVT): Defibrillate ASAP — biphasic energy per device; CPR immediately after.
Epinephrine: 1 mg IV/IO every 3–5 min (all rhythms).
Refractory VF/pVT: Amiodarone 300 mg then 150 mg; or lidocaine 1–1.5 mg/kg then 0.5–0.75 mg/kg.
Non-shockable: CPR + epi + reverse H’s/T’s (no defibrillation).
